Confusing Consultation!!

Hey Guys
Starting up on my own and having some problems with consultation forms!
The more I put on them the more I realise I have forgotton!!!!
Has anyone got any templates or form to which I cant adapt.
Any help or advice would be much appreciated!

Its for Facials,Waxing,ManiandPedi and Massage basically all beauty treatments!!
I will have a look at the creative cards as I do nail extension but have not invested in the products yet!! Thanksxx

If you belong to BABTAC have a look on their website they have a general consultation form and then you can use a separate for different treatments if you want.

I'm looking for some consultation cards myself.. i'm not a member of that thing mentioned sorry don't know what that is.. and mine would be for all beauty treatments, not really nails.
