I am trying to compile a consent form for the client to tick off and sign before their spray tan and wondered if there was a standard form that everyone uses? I thought I could think of most of them but my machine came today and in the box, their is a warning that the tube that feeds the tan through is made of latex and this got me thinking. you could go ahead and patch test clients but if they dont tell you that they are allergic to latex they could have a huge allergic reaction. How can I know that I have checked everything on the list and what do you actually put on your consent form?
