Consultation cards

I'm starting out doing treatments from home and will be making my own consultation cards, do you make clients fill a new card out each time or do you have space for them to sign at each visit once youv checked nothing had changed since the previous treatment? I'm unsure of how to set it out if I use the same consultation card each time they come? But would rather this than have to get them to always fill out new card. Any help would be appreciated! Thanks x
