Extending the nail bed with gel

I'm new to sculpting and currently practising. I'm fine using a form to sculpt when there is a free edge to put the form under, what do people do when there isn't? I've got a bad nail biter coming for extensions in a few weeks. I've searched the Internet for tutorials but I can only find ones which use L&P. I think I'm going to have to extend the nail bed but how do I do this with gel without it sticking to the skin?
 
Usually if they have no free edge I use a tip vs a form.
 
There's hardly any nail so I fear that if I use a tip there isn't then going to be any nail to adhere the gel too.

Cut out of much of the well as possible so it's literally just hanging on to the edge of the free edge. You can't extend the nailbed if there is nothing to build on. Its a tip or a form.
