Help Please - Client Record Cards

Hi Everyone

I have recently done a course through Artdeco and now want to do mobile make up artistry.

What do I need to have on my client record card? Like a disclaimer or something to say the client has no known allergies etc?

Any ideas would be much appriciated.

Hi im not mobile but work from home and my client cards have name address post code contact details number email address doc details allergies are they under doc supervision.. medication currently taken.... i think thats it off the top of my head??? hope this helps x
