Help with consent forms

hey all,

im looking at making my own flyers, business cards, client record cards and consent forms. the flyers and business cards seem to be straight forward but i need a little help on the client record cards and coonsent forms.

im booked in to do a course in feb but i want to have all the templates ready for when i start off as i can just go ahead and start spraying without having to wait on making these forms. could anyone help me as to what i should be putting on the consent form? i think i have the client record card ok, name, address, d.o.b, phone, skin type, comments, doc, doc num, patch test, date, allergies, :idea:

I print the Consultation back to back with the record card on stiff card so I can just turn it over on the clipboard (it's only for Men's waxing though - so you'd need to consider the content!).

The contra-indications on the front are based on a design from Training (thanks Andy!) and the record card is my own. I find the prices really handy since I can tot-up the bill without referring to more lists or bits of paper whilst the client is getting dressed.

I really wouldnt bother with the time and hassle of making them, you can get a pack of like 100 beauty record cards where the client has to sign from wholesalers website, salon serve is good and really cheap you would get everything you need from that catolague that you can order online.also vistaprint does loads of offers and is cheap and you can upload your own logo also. x
