Lash lift consultation form

Hi,

Does anyone happen to have a copy of their lash lift consultation form or any eye treatment form that would cover me for lash lifting and tinting.
Trying to make up my own as i can't seem to find any to buy... and want to make sure I have covered everything.

Nouveau sell packs of them for LVL but I havent seen what they look like for a few years. I've never used them tbh- I have my generic consultation form that covers all my treatments and the only thing specific to LVL/eyes is that it has a patch test/ tint test section on it.

If you don't fancy doing your own I thoroughly recommend Carl @ Verve designs for consultation forms. Mine are fabulous! colours and logos to your sepc, double sided and mine only cost £15! I have them saved to my pc and print off copies at home as and when I need them so no big printing costs either. xx
