Parental Consent Form

Hello,
Please does anyone have a parental consent form that they could send me? I have seen other threads but the poor geek who offered then received about a million requests and I didn't want to subject them to it again! Thanks

You just need the parent to sign the consultation form (and write parent next to sinature). You do not need a seperate parental consent form.
