permission letter for childrens parties

Hi
Can anyone help me with the wording for the permission letters to send out to the parents for the children's party's?

I want them to sound professional but this is the first party I have done?

Also does any of you geeks know where I can get the little bits to fill the goodie bags?

I have two party's booked one for eleven years old and one for 13 years old any tips would be great?
