Reflexology consultation form

I did a google search for reflexology consultation forms and printed off four or five that I thought were good and then adapted them to suit my needs. There is also ones in the reflexology books that might help. If you want a copy of the one I designed, pm me

I get my consultation forms through my insurance company FHT, does yours do the same ? (Unless you are with FHT)
