Skin testing form

Hey geeks,
does anyone have a copy of the form that they use for skin testing for clients to sign and what to put in the paragraph for them to read. I have searched the net but cant come up with anythink

I just get my clients to sign & date 2 identical form that I print myself as proof of skin testing, basically it talks about the importance of skin testing, and information about how long to keep it on their skin etc etc and date & time the test is applied. Simple really I keep one and they keep one. Then if the unfortunate was to happen I'm covered
