Spray Tan Record Card - please advise

Ok so I cant find anything online so I have decided to make my own spray tan record consultation form. Could one of you lovely geeks please advise if I have missed anything off? The cards that I bought have a space for GP and tel no but is this really necessary when you are working mobile. Surely if anything happened I would just dial 999?

Anyway here is my list of checkboxes

skin type = Sensitive, Fair, Medium, Dark
Have you had a spray tan before
Do you sufffer from any medical condition including Eczema, Asthma, HRT etc
Are you on any medication

I understand that I am responsible for notifying Karen of any medication I am taking or any skin disorders I suffer from before treatment.

On the back room for details of what was sprayed, date, etc

I would also add the following:

Do you require a patch test?

Are you pregnant or think you might be?

Do you suffer from asthma or any other respiratory problem?

Do you suffer from any allergies?

Are you currently undergoing any HRT treatment?

I would also put a bit at the bottom where they sign to say they agree to the spray tan treatment can be applied etc etc.....
