aero promotion invoice

Hi was just wondering what info I have to write on the invoice.
I know you have to put
your name
salon name
clients name
date of mini treatment on the vouchers but was wonering if I should put the same details for the invoice i.e
Mrs smith mini manicure 10th May 2008 1 voucher

how did you write your invoice as I dont want to get it wrong, and never had to write an invoice before.
