Can you let me know if this is ok. I am redesigning mine but this is only the first page so far with all the initial consultation details.

I am working on the treatment records next.:)

Is there anything I have forgotten?

Well this would be for all treatments, but obviously more specific questions would be asked depending on what they are having. I'm really trying to make a general consultation which doesn't turn into 'War and Peace'!!:lol:

btw not sure why the attachment is showing very clearly.:irked:
 
I got it up ok if you keep clicking on it and zoom in.
I think that it is fine as a general one.As you say you dont want to be going into their full medical history etc if they are only having a shape and varnish:)
 
I used to have a large consultation card. Now its very basic, I orally ask the questions and then tick yes to having CI or no, then elaborate on it.
You can tailor your questions to suit the treatment that you are giving.

I buy mine from Sally's and they are much like yours, mine are just for nails, and have the appointment schedule on the back, I like to keep it simple, just like me:lol: obviously when you are doing other treatments you need to take more into account but I personally hate forms so try to keep it down when it comes to clients don't want to put them of:lol:
