I am writing up a new client card and would like to include a release of all pictures so that I may use them for advertising and such. Does anyone have suggestions or a sample of the wording they use for this?

With your permission we would like to use photos of your treatments for future advertising and include within our portfolio. We kindly ask for you sign the following to grant permission to use your photos for these purposes.

ha ha sorry bit long winded there but something along those lines.
Alternatively you could end with if you do not wish for your photo's to be used please sign here.
