CND client record cards

Where can you get CND client record cards? I've checked on the CND site but they don't seem to have them.

Im not sure where you would go to get them in FL if your supplier doesnt have them but Holly aka fingernailfixer on site is a guru of all things CND in the states and I would guess would be able to tell you, so maybe send her a private message.
