Hello,

I know there are a few threads, but not really helping on certain questions..

Do you have seperate declarations / client forms for different treatments?

The standard client record cards in Capital & other wholesalers, are very general and do not have half of the questions you need to ask, & get signed for.

I was thinking to do seperate forms relevant to all treatments, and ask / look for the particular contra indications. This will remind you to look for them, and also, gives your client a chance to look at them to see if they have any of the conditions.

What does everyone do. I am sure, as I am, that everyone is quite hot on this sort of thing (Insurance, Client Signatures, Checking Contra Indication).

Can you help, so I know what to do?

I have one record card for each client with details of all treatments on the one card then it is easy to cross-reference and check what other treatments they have had in the past. I did my own on word and keep them in an A4 lever arch file in alphabetical order. I do make sure they sign each time they have a treatment though.
 
I did some research on different consultation forms and then did my own in word and printed them off.

I include anything they have on one form whether it's manicure, pedicure, enhancements or gel toes :)

Hi Carly

Well firstly, I was given a few examples when I did my foundation course but also I had one from college from my Mani/pedi course. Then I looked in my technician books, Encyclopedia of Nail and Complete Nail Technician and searched on here for old threads about consultations. Takes a while to go through them to see if they have any useful info in there but worth it for the tips that you get. I chose to include information about medication, condition and about client's lifestyle to give an indication of what stick the enhancements will get. Also included disclaimers for things like requested length, nail biting, medication etc., and additional space for details of each hand, e.g. tip sizes or mis shaped nails to refresh my memory.

It really needs changing now though as I'm learning from my own mistakes. One thing I would say was good is that someone suggested speaking to their insurers to go over their requirements for consultation. Obviously you don't want to miss anything that may exclude you from claiming on your policy should you need to.

Thanks for your response.

Basically, what I wanted to do was have the general client card, for basic details, and now that I have been up and running just over a year, start asking clients to check all details are still correct. Obviously, if something changes, like doctor address, they let me know, but for medication etc, I want this up to date.

I also bought a medical dictionary, as I want a better understanding of the side affects, and how it could affect treatments, so I need a declaration on this seperate sheet, so they can sign and say they agree to me looking up things and that all details are true.

What do you reckon? Any tips or help?

On my record cards I just keep basic info.. name, add, tel number etc. I attached my consultation form to it and it has a list at the back to put details of each visit, treatment and cost etc.

As far as the medical dictionary goes, no idea really. Never thought about going that deep into it. If client has a condition then seek gp's permission or if it's contra indication then adjust treatment accordingly or refer to gp.

Sorry I can't be more help. Maybe a more experienced geek can give more advice on this for you. :)
 
No that helps a lot, and thank you for your reply.

I will make up a form seperate to my client cards and adapt where I need to, which I think we should all do, depending on the treatment an clients job, lifestyle etc.

Just means a thorough consultation with everything recorded.
