Consultation form app

Hi I'm looking for app so client fill in consultation form online and that it stores it also .ideally forms that you can tailor too your needs with little cost. Thanks

I would use Google documents for this. You can simply share to client's email address and it will be stored as well. Once they filled out the form, I would change their option not to be able to touch anymore (for your safety).
Regarding signature I'm sure you can put a box on the document and we all live in a touchscreen life time so I'm sure they are able to provide some kind of signature as well.
You could also send aftercare advise along with a copy of this document.
This is actually a great idea as I'm a new business starter as well.

That ran through my mind too, so you think if they fill in the form online and sign an other document in person and I attach the two together that would be legally ok? Or signature must be on the client form on same page?

The signature has to be on the terms they're agreeing too, so would have to be on the same document. Even banks in recent years had to tighten up on this stuff.

Maybe I should build this as an app. A lot of people keep asking for it
