Consultation forms

Sorry for another extension post but was wondering where all you experienced extensionists get your consultation and disclaimer forms printed off from for hair extensions? Thanks in advance xx

I've probably worded it wrong im not sure :/ my hair extension trainer told me she has a separate disclaimer sheet to write notes on about things that have been said that aren't on the consultation form to cover her back if things go sour so to speak with any clients hope I've made a bit more sense there

I get mine from the hair company I use.6/10 have their own you can buy.. I also have made my own and use my iPad for it..x
