Consultation forms

Hi i'm opening my beauty salon next Tuesday.
I'm struggling to find consultation forms.
Does anyone have one they could let me copy from.
I'm doing facials, eyelash and eyebrow treatments, waxing, manicure and pedicure.

Most beauty wholesalers sell consultation cards, if this is of any help.

Were you not given examples of consultation forms where ever you did your training ?
i ask this as looking at your profile shows no training information at all ?
