Has anyone got an example of an under 16 consent form to do Shellac nails

Hope you can help , I have been asked if I would do a 15 year old nails using Shellac. I would feel better if I get the parent to sign a consent form, but don't know where to start. If you have any examples of what you use I would be really grateful.

Could you not do one up yourself? Then it'd be personal to all you want to apply.
Just an idea.
Then if you have more under 16s you'll be well equipped, I'm sure with warmer weather coming the youngsters will be in with their mums etc.
