Keeping track of patch tests

How do you keep a track of people who have had a patch test.do you give them a card or keep a record yourself?

I have a book like an address book

Each new client gets a record card with all their details on, if a new colour client comes in I make her or him, a card and log the patch test on that along with whatever chemical treatments that've had and date them

I have a consultation form for every client and if they have a patch test done for anything, there is a space or me to make a note - when, what etc!

Patch tests are recorded on client record cards. Where they sign etc - insurance requirements for me.
