I have to do a sample letter to a clients gp regarding contra indications for a assignment, any ideas?

I always think that plain English i sthe best way.

Dear Dr X,

Your patient, Mrs Smith has asked me to carry out xxxxxxxx treatment. However, I am concerned that she appears to have a rash/lump (don't diagnose) on her (name part of the body). Could you please confirm in writing that it is ok for me to go ahead with the treatment.

Thank you in anticipation.

Just to add you could do a return slip at the bottom for the dr to sign and send
back. just an idea.

I am a professionally qualified and insured nail technician. My business is to change the appearance of client’s natural nails by the application of artificial nail enhancements. The enhancement products give a permanent coating of plastic polymer that is chemically bonded to the natural nail plate.
Your patient Mrs._______________ has expressed a wish to have nail enhancement services from my salon. I have advised her to obtain your permission prior to any treatment.
Would you please send a letter with your patient confirming that it is acceptable to proceed with the artificial nail service?

As Ruth has put in the above thread, i always put in what the treatment is and what it involves, that way there is no confusion. A reply slip, and a stamped addressed envelope will usually get you a reply as well.
