Waxing consent

Hi. I offer waxing services from home and have a waxing consent form for my clients to fill-out and sign. Just wondering if it's important to also get their signature each visit confirming that they haven't changed products or medications. Thanks for any input.

I don't see why you would have to make the client sign each visit. The initial form informs them of the need to disclose any medication (especially acne meds) that would make the skin thinner.

If I haven't seen the client in a while (like a year) I ask them to update the form.
 
I always get them to initial to say nothing has changed and that they have received after care. If you didn't get them to sign and god forbid something happened to them on your table then it could open up a whole can of worms. A simple signature is no hardship to cover your own back.
You could always ask your insurance company what they require you to do,also.

Me too! I get them to sign again everytime I see them to ensure that nothing changed since the last visit. Its amazing how few clients remember what they should be telling you, especially those who just found out their pregnant and are keeping quiet until 13 weeks. They often do not realise some oils etc we use could be harmful.

I have had to force it out of some clients who would never have told me if I hadnt asked them to resign the form.

In my eyes and even with the disclaimer that they need to inform me of any changes, it backs my professionalism up as well as confirms I have done all I can to ensure that the client is safe to have the treatment she is booked in for.

get there signature babe it only takes a second, I had a lady for reflexology in march she came back to me in may saying her feet were burning constantly on her instep, which is the spinal area I had asked her if she had ever had, or now has any back problems to which she replied NO!!! I asked her twice and had another therapist with me for consultation to write notes as she was observing the treatment. She said she had been to the dr and he had said it was because of the reflexology so i asked her for her dr's name and telephone number and she wouldnt give it. I told her that any adverse reaction would only last 24-48 hours and not two months.

I asked her again have you ever had any spinal problems to which she now replied oh yer i had spinal surgery in 1989 ........ so i got out her record card and sure enough she had signed to say she didnt have any spinal contraindications and she had intialed every where i had asked her a question regarding contraindications so i had covered my own back. Three times she came back in and three times she demanded a free treatment as she was adamant that I had made her feet burn....... I declined her demand for a freebie but she has been back for further treatments.

morale of the story is make them sign and cover your own back

It always amazes me how these clients don't realise the importance of telling you this information all for the sake of getting through the door to have a treatment :eek:
And for reflexology! Did she think you would not detect anything from her feet? lol I do get quite firm with these clients as it really annoys me xxx

some people would do anything for freebies especially this day and age
 
Nom just get them to sign first time and say on your form that they must advise you of any changes in health and medication. xxx
 
On each and every visit I ask my client whether anything has changed on their card since their last visit and following their treatment I ask them to initial on every visit, this for me is a client endorsement that they are happy with the service they have received, and is also a confirmation that they have received their aftercare.

In these days of "sue em" culture, CYA in my book - that's (cover your a**e).
