Client Record Cards/Consultation Cards

Hi everyone,

I was just wondering what you use? When I first started out (Dec 08) I bought some client record cards from Sallys. I actually think now their pretty rubbish as its got nothing to take informnation about like allergies/medication etc... and nowhere for them to sign. Do yours have this? Whilst we've been doing clients for assessments at college, they've got CND record cards and they look great, its got a bit to put all their info...like name, address, phone number etc.. Then its got a section for any allergys, medication, illnesses etc... Then its got a bit about the health of their nails and then a bit for them to sign. Is this what they should be like? I was thinking I maybe able to get some from college or just order some from Sweet Squared, but thought I would ask your opinions and listen to what yours are like. Also, do you get a signature from every client for every treatment or just new clients? Even when I get new clients and I ask them to sign, they always look at me a bit wierd as if I shouldnt be asking them for it!!

Well practically the whole world uses this one that I did for my salon years ago. If anyone wants me to send them an attachment for printing on their computer then email me at [email protected] and I will be glad to send it to you. The back of the CRC has a complete record of client visits too for approximatly 1 year.

Hehe thats the one I use but I have added work and lifestyle questions to it...

I buy the ones from sweet squaered (CND) as everything is on there and they look nice and professional
