consent form for under 16s????

:lol: hi can anyone help if i do a manicure and nail art for the under 16 i need a parent to fill out a consent form explaning the treatment etc does anyone else do this??? could you maybe e mail me or pm me your consent form to give me some ideas??

I have chosen to get consent from under 16s. The reason is if something happens and the parent is not happy they will come back to you. You can either phone the parent and get them to write a note to say they are happy for the treatment to go ahead and date and sign it. Or ask the parent to come with them for the treatment.
