Downloadable ITEC case study form???

Firstly, Happy New Year Everyone! :hug:

I'm not sure if anyone can help.
I'v hand written all my manicure and pedicure case studies, my tutor told me before the Christmas holidays that I can download a template/form from the ITEC website so I can type them all up.
I've looked for it on the website..& I can't find it anywhere
I really need to find it so I can hand them in when I go back to college.

Can anyone help?

On the first link..theres a sample case study....
thats what I need but without the all the details...just plain with the headings.

hi girl, its the Sample Client Consultation Form that you need. and any way there wont be enough room to type all the details in. what i did was just used the headings and then typed them out.i think i still have them if you want i can send them to you by email. pm me if you need any more help
