I made up my own consultation card as I wanted it to be ultra thorough! I also get my clients to sign a disclaimer!

Which ever company you did your training with for the extensions should have them to retail to you,

Just a quick hint, if you are doing your own, just make sure that they contain all the information, questions and recommendations that you need but also that your insurance might require.
I would have thought it will be a lot easier and quicker if you contact the company you have been trained with or any of the others mentioned on here as that will be the safest way?:hug:

its now all sorted, i rung the wrong number opps!!! lucky my client was ill today!! means i got more products, and info on the lashes i do for the clients x
