waxing specific client record cards

I've searched but can't find the answer, can someone tell me where I can get waxing specific client record cards? My Capital don't do 'em, that's where I normally get everything!

Just make up your own. I find it much better as it allows me to add a section for the client to sign each time they come in to say they have had no changes to their health etc.

I made my own aswell that i added a section to sign before every treatment to say nothing has changed.
