Wording parental consent form

hi geeks , i was wondering wether anyone could help me word my u18 parental concent form for beauty treatments

im not very good with words

i was thinking along the lines of
as parent/guardian i give my written consent for (therapist) to perform (beauty treatment) on (childs name)
i take full responsibilty for any decisions made before/during/after treatment has taken place and have been informed by (therapist) of any contra-actions/indications that may occur during/after treatment.

I'd run it past your insurance company, just in case they have any specific conditions relating to treating children (age limits etc.).
