Young lady, nail extensions?

some people might disagree, I have a young lady booked in on tuesday for nail extentions, she has previously bin havin GelFX on which is just a basic gel manicure..

However her mum will be there on tuesday, When doing the consultation, and filling the client card in would you get the mum to sign or the daughter?

Age, is around 14.
Some people out there might dissagree on doing someone so young, but shes a regular client and didnt want to let them down? Thanks :D
 
Mum needs to sign definitely as she is parent x x I wrote out a form for situations like this where you sign to say you have listened to parents specifications (if thats the right wording) etc and then a place for mum to sign to say she consents and understands any contra-indications/actions etc x x
 
Surely a form should already have been filled in if she has been having GelFX?
Also, does your insurance company cover you for under 16's?
