CND Client Record Cards

I received a pack of the CND Client Record Cards that I had ordered a few weeks ago. I had a brief look at them when they arrived and put them away as I am only just setting up my home salon and so haven't needed them yet. I decided to look at them in more depth today and have realised that they don't ask about medical history or whether the client is taking any medication. Also the area that has to be signed is only applicable to clients having enhancements. The CND Manicure and Pedicure training booklet also shows the same Client Record Card. Does anyone else here use these for all nail services or do you create separate ones for Manicures and Pedicures e.t.c ?

Just looking on the S2 website and the Client Record Cards in the picture are very different to the ones that I have been sent. I will have to call them tomorrow and ask if I have been sent an old version or something :sad:
