But does anyone have a sample disclaimer form that gives me an idea as to what goes on there??

I am doing my own, and I am affraid that I miss anything out.

if you have a supplier like sally's near you i've seen them in there so you could have a look at them and get an idea of what to put

What holds water is the fact that having signed this the client has acknowledged that these issues have been brought to their attention and the salon has a record of it in case at some future date for any of the above reasons, there is a problem that can be attributed to any of the factors mentioned.

I think this is okay, but you need to change the last sentence, as it seems to negate you of all responsibility:).
How about.....

I think as we are often asked in the salon to rebalance over another technician's work, it is important to let the client know that we will not be held responsible if something breaks down where the other technician worked ... we don't know what the last technician's prep was like or her sanitation which is why I won't guarantee anything if working over another's nails.

I 'think' this thread is about a disclaimer to stop us promising any guarantee for our work when the client has came to us with less than perfect nails and/or nail products from another therapist and is nothing to do with client record cards, as required by your insurance... Although the two can be combined:)
