Tape in extensions?

I am a bit freaked out on this and wondering whats the prob? Need help

I installed tape in extensions on a girl 2 days ago. She came to me today with her extensions all out. She then said big chunks of her hair were coming out...so I said...so all your extensions fell out. Shes like no they all stayed there. So I said did they go lower. Shes like no they didnt budge. She then said her hair not where the tapes were decided to start coming out of her scalp. Could this be a reaction? If so I would think the tapes would of fallen along with her hair but it all stayed. Can anyone give me suggestions? What do I do with this. I cant see her head cause they are all out now

Could you see bald patches? Redness, swelling and itching would all be signs of a reaction. Possibly even a rash?

Maybe they were too close to her scalp and was pinching the scalp
