Wording to allow e-mail etc

Hi,

I am looking to add wording to my consultation forms which will allow the client to be contacted by text/ email etc.
Does anyone use this at the moment? and if so is it effective?

Would anyone be able to suggest suitable wording?

Many thanks as I am stomped....
 
I have put on mine:

"Please tick here if you do NOT wish to receive information on future Nails By Knight special offers and services" and I have added a little tick box next to it.

Seems to do the trick

Also you might want to discriminate between being contacted via text or email for marketing/offers etc, or for things like appointment reminders etc.
 
Email:____________________________ You may be contacted via email for .... (state your reason for wanting this info). If you do NOT wish to be contacted via email please tick box [ ]
Mobile: __________________________ you may be contacted via text for .... (state your reason for wanting this info). If you do NOT wish to be contacted via email please tick box [ ]
I also have a note saying:
This information is for correspondence only. No confidential information about you will be issued to any company, organisation or others.
