Eyelash extensions - consultation. Which is better?

Hey ladies

Over the past year ive been changing and fixing my consultation forms for eyelash extensions (sp). I cant decide which one to use?

Woul appreciate if you had a look. If you have any pointers or advice - feel free to tell me

I tried to upload pdf but couldnt then upliaded these and noticed that they are unreadable... after i posted the thread! Now i cant delete it as i know it makes no sense to have it here when nobody can read the text! Lol

I would say the image on the left is alot more clearly for clients, would definetly recommend that image.

True, but it depends if all the necessary information is included on that form.

Your right, as long as the information is included as you said, the layout of the left consultation form is more practical.
