Where can I find a copy of a 'Parental Consent' form?

Do any of you lovely geeks happen to have a copy of a Parental Consent form that I could take a look at?

I have a 13yr old coming in next week for eyebrow waxing.

I have only ever done stuff to family members who are under 16yrs, so I've not had the need to have one.

I am capable of drafting it up on the pc, but I was looking for a few pointers. I also guess I could leave a blank space or have tick boxes for which service I am going to need consent for.

Any help on the would be very much appreciated.

I would just make a written note on the client record card about the childs age and that her parent is giving her permission for the treatment to go ahead and let the parent sign and date her signature and your comment.

I personally don't have any seperate parent consent forms... :hug:

Same here ..i would just ask usual questions as i would a adult during consultation but have the parent sign\date and add comment.

I get the parent to fill the record card in, in their childs details and sign as the parent.
