Hi Geeks,

Following on from recent threads and personal feedback on training, I thought it might be useful to share waxing paperwork.

I have attached a basic consultation form and aftercare leaflet based on the ones I created for Perron Rigot at Professional Beauty. These are pretty much identical to the ones I use in my own salon, only smaller and without my salon logo or contact details, and are an amalgamation of bits and pieces from various textbooks, websites, Habia material and student and client feedback over the years.

I'm not suggesting for one moment that these will fulfill everyone's needs forever and ever amen. You will probably want to add things to meet the requirements of your own working practice or insurance provider, and you may also want to reword or take things away, but I hope you will find them a useful starting point.

Please feel free to print, adapt or borrow for your own non-commercial use (i.e. don't try and flog them as an Ebook on Ebay like someone did with one of my waxing tutorials!). If you are going to use them in a college assignment or something similar, please do the decent thing and acknowledge Salongeek as the source.

I'd be interested to see what other Geeks use or suggestions for changes.

Thanks v much for these. I can truthfully say that in 25 years of being waxed I have never, not once, been given any such form.
When I complete my training with Kim you can be sure that I will ensure that each and every client will be given such care.

when it comes to waxing Andy knows his stuff !!!!! he is a walking wax encylopeadia. If he was ever on mastermind with waxing as his specialised subject he would walk it !!!!!

all you geeks out there that offer or who are embarking on waxing get these and download as you will find no better !!!

I am a wee bit confused here, which isn't hard.

Do you mean if a client indicates they have a problem on the consult and you go ahead anyway?

On my consultations for all treatments I ask all the questions and mark Y/N and the client signs it.

I don't consider this a waiver, if a client has answered yes to a condition and is contra indicated I refuse treatment and mark it accordingly on the card, and offer if possible another treatment that they are not contra indicated for. This is exactly what Andy also reinforced at our workshop on Monday.

The way I have read this bit is that a Consultation is a Waiver, which is not what I use it for.

EDIT: A few messages have been deleted since this was first posted, but a good point had been raised about waivers and the advice still stands.

Put simply, your consultation form cannot be used to waive the rights of your client. What it can do is help you make an informed decision about the proposed treatment, i.e. whether it is safe and appropriate to wax, whether you need to adapt your treatment plan, or whether you need further information or approval from the client’s healthcare practitioner before proceeding.

The declaration at the bottom of my consultation form serves 4 purposes:
  • The client signs to say that they are telling the truth – after all, the safety and efficiency of our waxing service relies in part on the information we are given by that customer
  • It serves as confirmation that I have informed my client about the expected results and side effects of waxing – e.g. that waxing isn’t a permanent method of hair removal, that they may have some soreness or irritation afterwards, etc etc
  • The client agrees to follow the aftercare advice I give them – if they don’t, this is at their own risk
  • With all the facts in hand, the client is then able to make an informed decision and gives (or declines) their permission for me to wax them – this is not a “go ahead and wax me even if my skin is peeling off” disclaimer, that final decision and ultimate responsibility still comes down to me as the professional!
Remember that your written consultation is just one step of the process – use it alongside your verbal questions, plus your visual and physical examination of the skin to help formulate a treatment plan that is right for that individual.

As always – if in doubt, don’t wax.

Hi I have designed my own client cards as there would be too many for all treatments. I include not a waiver but a paragraph on informed consent. This means that if the client is not contra-indicated and proceed they know that some treatments have temporary side effects. This I believe is what Andy is trying to say. Further info on particular treatments discussed is also added to the treatment log.
informed consent: I understand that after some treatment procedures the following observations may occur: Minimal skin discomfort for a short duration of time - Dry patches may appear with slight flaking of skin - Temporary skin reddening may occur - Possible breakouts which can be managed with home care
It is recommended to follow home care advice and to use the appropriate home care products.

Quick question for you, on the consultation, there is a bullet point that says "Use or Roaccutane or other acne products in the last six months". Is there another product missing before "or Roaccutane", or should the "or" be "of" ?

In my insurance (BABTAC) terms and conditions - it says that you must give suitable WRITTEN aftercare at each and every appointment.
